Intake forms

The questions you always ask, answered before arrival

Intake forms are questions owners answer about each pet.

Applies to the booking types that use the form
How often: Once per pet or Every booking
Questions with an Answer type, Required, and Show only if... so a question appears only when an earlier answer matches
Require approval before booking: staff review answers, then approve or decline
Change the questions and owners who already answered are asked to answer again

Waivers

Signed, versioned, and not lost

A waiver holds the agreement customers sign.

Applied to Booking, Boarding, Daycare, Mobile grooming, Walk-in or Product/service
Terms and services with an Effective date, kept as versions so earlier signers stay signed against what they saw
Sign on each visit, or once per version
Choose which locations use it, and Restore a deleted waiver

Safe by design

Nothing is lost

Switching a form from Once per pet to Every booking deletes nothing; earlier answers come back if you switch back.

Chains

A location shows a waiver as Read-only when it was created at another location or the parent company.

Booking gate

A pet whose required form is not approved cannot be booked for the booking types it applies to.

Questions about forms and feedback

Can a form stop a booking?

Yes, if you switch on Require approval before booking. A pet then needs that form approved before it can be booked for the booking types the form applies to, and a declined form always blocks booking.

What if I change the terms of a waiver people have signed?

Add a new version with a new Effective date. People who signed the old one stay signed against what they saw, and customers sign the new one when it takes effect.

Can customers sign on every visit?

Yes. Switch on Sign on each visit, or leave it off for once per version. It is locked on when the waiver applies to a Product/service.

How do I connect surveys?

Open Settings, Customer, Listen360, switch on Enable Listen360 Integration, enter your username and password and save.
